Bakersfield PACE is not intended to replace family nor is it structured as a long term care facility. Instead, we take a ‘team approach’ to care at the PACE center partnering with the participant, the caregiver and family to enable our participants to remain in the secure surroundings of their own home environment.

The Interdisciplinary Team collaborates to build an effective plan of care to ensure that medical, social and emotional needs are met. The Team consists of but is not limited to:

• Primary care physician
• Nurses
• Social workers
• Activities Director
• Registered Dietician
• Medical specialists by appointment:
  Dentist, Audiologist, Podiatrist, Optometrist
• Home Safety and Home Care nurses
• Personal Care Attendant or other caregiver
• Transportation staff
• Occupational, Physical and Speech Therapy

Once you enroll in the Bakersfield PACE program, participants agree to receive all of their healthcare through the program. Instead of feeling limited, most people are pleased to discover they receive a more complete, balanced and personal approach to their care than ever before.
